At Gordon Physical Therapy, we put our clients first. From the moment you walk through the door, we are here to answer your questions and to make you feel comfortable with the care you are receiving. From knowing how your therapy sessions will progress, to understanding how the billing process works with your insurance, keeping you informed and comfortable while in our care is of the utmost importance.
We are in-network providers for most major insurance plans, as well as Workers’ Compensation claims and motor vehicle accident claims. We also offer cash-pay services for those clients with out-of-network plans. We are in-network with the following insurance plans:

NEW CLIENTS
Welcome to Gordon Physical Therapy! As a new client, it is our goal that you feel comfortable with the care you are receiving and that you know exactly what to expect from us.
Please fill out the “New Client Forms” prior to your initial appointment (this will save you some time in the clinic). If you don’t have time to complete the forms prior to your evaluation appointment, we’re happy to provide these forms for you in the clinic.
Lastly, you can view our “Notice of Privacy Practices” if you’d like. However, you don’t need to print these forms out, unless you’d like them for your own records.
If you have these filled out before your evaluation visit, please arrive 15 minutes before your appointment so that we can get them processed. If you need to complete the forms in our office, we ask that you arrive 30 minutes early to ensure adequate time can be spent with your physical therapist.
Be sure to bring your referral paperwork (if required), as well as photo identification and insurance information.
Once your appointment has begun, you will have a thorough examination by one of our physical therapists in order to determine precisely which services you need to help you reach your goals. This exam will take approximately 40 minutes, and you will need to be wearing comfortable clothing so that you can move easily. Your total treatment time may last up to 60 minutes, if you and your therapist decide that heat or ice will be beneficial. At the completion of your evaluation, your therapist will inform you of the results of your evaluation, including the nature of your pain or injury. You and your therapist will also work together to develop your therapy goals and treatment plan.
RETURNING CLIENTS
If you’re a returning client, welcome back to Gordon PT! Please arrive at the clinic 15 minutes before your appointment time to update your paperwork. And please let us know if you have changed your address or phone number, or if you have additional insurance information to update.
